No death certificate is requested. Have the deceased's name and address, the account number if you have it, the date of death, the executor's contact details and a meter reading if possible.
- The deceased's full name, date of birth and last address
- The date of death
- Account, policy or reference numbers if you can find them (old statements or emails)
- Your relationship to the deceased and your own contact details
- The account number and the address the service is supplied to
- Somewhere to note any reference number they give you
